Nurse SENTENCED After FBI Exposes $69M Medicaid Scheme Targeting Native Americans

A nurse practitioner from San Tan Valley, Arizona billed the state's Medicaid program more than $69 million in less than a year. The state paid her clinic almost $55 million. Prosecutors say sober home operators were paid to send her patients, many of them Native Americans recruited from tribal land and driven to Phoenix. After a federal subpoena arrived, she told former staff to write therapy notes for sessions that never happened.

In May 2025, Rita Anagho pleaded guilty to conspiracy to commit health care fraud and wire fraud. In September 2026, a federal judge sentenced her to 14 years in prison. She was ordered to repay almost $55 million and forfeit about $9.5 million from seven bank accounts, along with nearly $7 million in real estate. The FBI and the HHS Office of Inspector General investigated the case, with help from AHCCCS investigators.
